Job Summary

Position Title: Medical Assistant, Integrated Care Coordinator

 

Position summary: The Medical Assistant, ICC supports daily clinical and operational workflows across multiple service lines including primary care, specialty care, urgent care and health home programs. This position ensures patients receive compassionate, holistic, and well-organized medical care by assisting providers for in house testing while also coordinating support for the patient overall well-being. 

Responsibilities

  • Perform patient triage to assess and prioritize medical needs. 
  • Assist healthcare providers with in-house procedures such as EKGs and PFTs. 
  • Ensure examination areas are sterile, organized, and properly equipped. 
  • Sterilize and autoclave medical instruments; uphold infection control protocols. 
  • Build and maintain positive relationships with patients, caregivers, and care teams. 
  • Conduct in-person assessments, and surveys to evaluate medical and social needs. 
  • Develop and implement individualized care plans in collaboration with providers. 
  • Promote timely access to care and increase preventive care utilization. 
  • Schedule medical appointments and coordinate transportation. 
  • Educate patients on community resources and refer as needed. 
  • Act as a point of contact, advocate, and resource for patients and families. 
  • Maintain detailed, up-to-date patient records and coordination logs. 
  • Develop, update, and maintain individualized care plans in collaboration with patients, providers, caregivers, and the care team. 
  • Support patients after emergency room visits, hospital admissions, inpatient discharges, or other transitions of care. 
  • Connect patients to community resources that support health, safety, housing, food access, transportation, benefits, and overall well-being. 

Qualifications

Required Qualifications: 

  • Certification as a Medical Assistant (CMA or RMA) preferred. 
  • Associates Degree 
  • Strong understanding of infection control and clinical protocols. 
  • Excellent interpersonal, communication, and organizational skills. 
  • Ability to work collaboratively across clinical and administrative teams. 

Required Skills:  

  • Competency in conducting triage, assisting with medical procedures, and performing sterilization and autoclaving. 
  • Excellent communication skills to interact effectively with patients and healthcare providers. 
  • Strong organizational abilities to manage documentation, and logbook updates. 
  • Attention to detail is crucial for accurate documentation and maintaining a sterile environment. 
  • Ability to work collaboratively with healthcare professionals in a dynamic medical office setting. 
  • Flexibility to handle diverse responsibilities and adapt to changing priorities. 

 

Physical Requirements:  

  • Hearing: Adequate to perform job duties in person and over the telephone. 
  • Speaking: Must be able to communicate clearly in person and over the telephone. 
  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens. 
  • Requires frequent bending, reaching, standing, walking, squatting, and sitting, pushing, and pulling exerted regularly throughout a regular work shift. 
  • Prolonged periods of sitting at a desk and working on a computer.
