Job Summary

We are seeking a Board-Certified or Board-Eligible Neurology Physician (MD/DO) to provide on-site neurology consultations for residents in skilled nursing and long-term care settings. The physician will evaluate and manage complex neurologic disorders while collaborating with Advanced Practice Providers and facility teams.

Responsibilities

• Provide on-site neurology consultations in SNF/LTC settings

• Diagnose and manage neurologic conditions including stroke, epilepsy, Parkinson’s disease, dementia, neuropathy, and movement disorders

• Perform comprehensive neurologic examinations and cognitive assessments

• Order, interpret, and act upon diagnostic studies including imaging and laboratory tests

• Manage neurologic medications and treatment plans

• Supervise and collaborate with Nurse Practitioners and interdisciplinary teams

• Participate in care planning, quality improvement, and hospital readmission reduction initiatives

• Ensure accurate and regulatory-compliant documentation in EMR systems

Qualifications

• MD or DO degree from an accredited medical school

• Completion of an accredited Neurology residency

• Active and unrestricted New York State medical license

• DEA registration

• Board Eligible or Board Certified in Neurology

• Experience in SNF/LTC or post-acute care preferred
