Job Summary

We are seeking a compassionate and dedicated Cardiology Nurse Practitioner (NP) to provide on-site cardiovascular care to residents of skilled nursing and long-term care facilities. The Cardiology NP will collaborate closely with cardiologists and interdisciplinary care teams.

Responsibilities

Responsibilities include:

  • Provide on-site cardiology consultations including initial and follow-up evaluations

    · Assess and manage cardiovascular conditions such as hypertension, CAD, heart failure, arrhythmias, and valvular disease

    · Order, review, and interpret EKGs, echocardiograms, labs, and diagnostic studies

    · Collaborate with cardiologists and interdisciplinary teams to develop treatment plans

    · Manage and titrate cardiac medications per evidence-based guidelines

    · Educate patients and caregivers on disease management and lifestyle modifications

    · Ensure accurate EMR documentation and regulatory compliance

    · Participate in quality improvement initiatives to reduce hospitalizations

Qualifications

Minimum 6 months experience in SNF/LTC or hospital/outpatient cardiology setting

· Active and unrestricted Nurse Practitioner license

· DEA license Recommended

· BLS/CPR certification

· Master’s degree in Nursing from an ac credited NP program

· Preferred cardiology experience (training available)

· Strong communication, organizational, and time management skills
