The Senior Manager will lead community outreach, partnership development, and oversee the NYREACH Navigator team to drive program growth and member engagement. This role is ideal for someone who can operate both in the field and internally—building relationships externally while ensuring strong execution and performance from the team.

 

The Senior Manager will be responsible for expanding NYREACH’s reach through strategic partnerships and outreach, while directly managing the Navigator team responsible for screenings, referrals, and member engagement.

 

This role connects strategy to execution, ensuring outreach efforts translate into completed screenings, successful referrals, and meaningful outcomes for members.

Job Summary

Job Summary: As the Executive Director of NYREACH, the ideal candidate will play a critical role in NYREACH’s fundraising efforts, management of administrative day to day operations, program oversight and hiring/managing a capable staff who, along with the ED, will carry out the mission and vision of the organization to profound success. The ideal ED will have expertise or significant experience researching, developing and maintaining programs in community health and workforce development; be well versed in key areas of healthcare operations; have experience applying for and managing grant/funding opportunities through private or gov’t contracts; and have experience managing and mobilizing a staff toward success while effectively reporting to a Board of Directors.

Responsibilities

Community Outreach & Growth Strategy

 

· Develop and execute outreach strategies to increase awareness and utilization of NYREACH programs

· Identify target populations, housing developments, and high-impact community areas.

· Lead and oversee community events, tabling efforts, and local engagement initiatives

· Drive consistent member engagement through outreach campaigns and referrals

 

Business Development & Partnerships

· Build and manage relationships with health plans, healthcare providers, CBOs, faith-based organizations, and local businesses

· Identify and secure new partnership opportunities that drive referrals and program growth

· Support partnership agreements and collaborative initiatives

· Maintain ongoing communication with partners to ensure alignment and performance

 

Navigator Team Oversight & Performance Management

· Directly oversee NYREACH Navigators responsible for screenings, eligibility assessments, and referrals

· Set clear daily and weekly production targets (e.g., screenings, navigations, referrals completed)

· Monitor performance and ensure quality, accuracy, and timely documentation

· Provide coaching, training, and ongoing support to improve productivity and outcomes

· Ensure compliance with SCN requirements, documentation standards, and reimbursement guidelines

 

Program Growth & Conversion Management

· Track performance across the full funnel: outreach → screening → referral → service delivery

· Identify gaps in conversion and implement strategies to improve outcomes

· Align outreach efforts with navigator capacity to ensure efficient service delivery

· Support scaling of successful outreach and navigation models

 

Cross-Functional Collaboration

· Work closely with outreach teams, clinical teams, and operations to ensure seamless member experience

· Coordinate with internal teams to align outreach efforts with available services and capacity

· Partner with marketing to support messaging, materials, and campaign execution

 

Reporting & Strategy Support

· Provide regular reporting on outreach, navigator performance, and partnership outcomes

· Track key metrics including screenings, referrals, completed services, and partner contributions

· Support leadership with insights and recommendations for growth and expansion

Qualifications

 

Required Qualifications

· Bachelor’s degree required; Master’s degree preferred

· 3–6+ years of experience in community outreach, healthcare, non-profit, or related field

· Experience managing teams, preferably outreach or navigation staff

· Proven ability to build partnerships and drive program growth

Preferred Experience

· Familiarity with Social Determinants of Health (SDOH) and Social Care Network (SCN) programs

· Experience overseeing navigator or care coordination teams

· Bilingual (Spanish) preferred
