The Senior Manager will lead community outreach, partnership development, and oversee the NYREACH Navigator team to drive program growth and member engagement. This role is ideal for someone who can operate both in the field and internally; building relationships externally while ensuring strong execution and performance from the team.

Job Summary

The Senior Manager will lead development, community outreach, and field operations to expand NYREACH’s footprint across New York City. The focus is to build strong partnerships, drive consistent referral pipelines, and ensure outreach efforts convert into real member engagement and completed services.

Responsibilities

Business Development & Strategic Partnerships

· Build and manage relationships with health plans, provider groups, CBOs, faith-based organizations, and local stakeholders

· Identify, secure, and grow partnerships that generate consistent referral volume

· Develop partnership strategies aligned with NYREACH growth goals

· Lead partner engagement, performance tracking, and joint initiatives

 

Community Outreach & Market Expansion

· Design and execute outreach strategies that drive awareness and engagement

· Identify high-impact geographies and populations

· Oversee community events and field engagement with focus on conversion

 

Navigator Team Oversight & Performance Management

· Directly oversee NYREACH Navigators responsible for screenings, eligibility assessments, and referrals

· Set clear daily and weekly production targets (e.g., screenings, navigations, referrals completed)

· Monitor performance and ensure quality, accuracy, and timely documentation

· Provide coaching, training, and ongoing support to improve productivity and outcomes

· Ensure compliance with SCN requirements, documentation standards, and reimbursement guidelines

 

Program Growth & Conversion Management

· Track performance across the full funnel: outreach → screening → referral → service delivery

· Identify gaps in conversion and implement strategies to improve outcomes

· Align outreach efforts with navigator capacity to ensure efficient service delivery

· Support scaling of successful outreach and navigation models

 

Cross-Functional Collaboration

· Work closely with outreach teams, clinical teams, and operations to ensure seamless member experience

· Coordinate with internal teams to align outreach efforts with available services and capacity

· Partner with marketing to support messaging, materials, and campaign execution

 

Reporting & Strategy Support

· Provide regular reporting on outreach, navigator performance, and partnership outcomes

· Track key metrics including screenings, referrals, completed services, and partner contributions

· Provide insights and recommendations to leadership

Qualifications

 

Required Qualifications

· Bachelor’s degree required; Master’s degree preferred

· 3–6+ years of experience in community outreach, healthcare, non-profit, or related field

· Experience managing teams, preferably outreach or navigation staff

· Proven ability to build partnerships and drive program growth

Preferred Experience

· Experience working with Medicaid and managed care experience

· Familiarity with Social Determinants of Health (SDOH) and Social Care Network (SCN) programs

· Experience overseeing navigator or care coordination teams

· Bilingual (Spanish) preferred
