Position Title: Nurse Practitioner – House Calls

Job Summary: Intention Healthcare is seeking a compassionate and skilled Nurse Practitioner (NP) to provide high-quality, patient-centered care through home visits in the Greater Boston region of Massachusetts. This role focuses on individuals with chronic illnesses, mobility challenges, or limited access to healthcare facilities. The NP will emphasize holistic care, patient education, and management of complex medical conditions, ensuring continuity of care for patients in their homes.

Responsibilities

Responsibilities include:

 

· Patient Evaluations: Conduct comprehensive health assessments during home visits, including physical exams, medication reconciliation, and review of medical histories.

· Diagnosis & Treatment: Diagnose medical conditions, develop individualized care plans, and prescribe medications as appropriate.

· Care Coordination: Collaborate with physicians, specialists, and other healthcare professionals to ensure integrated and continuous care.

· Patient Education: Educate patients and families on disease management, lifestyle changes, medication adherence, and preventive care.

· Documentation: Maintain detailed and accurate electronic health records (EHR) of patient assessments, treatments, and follow-ups.

· Follow-Up Care: Monitor patient progress through follow-up visits and adjust treatment plans as necessary.

· Emergency Response: Assess and manage urgent medical situations during home visits, providing timely interventions or referrals.

· Community Engagement: Participate in local outreach initiatives to promote health services and educate the community about available resources.

Qualifications

Job Requirements:

  • Master’s Degree in a Nurse Practitioner program (Family or Adult Geriatrics).
  • Current Massachusetts Nurse Practitioner license required.
  • National Board Certification through one of the following:
    • American Association of Nurse Practitioners (AANP)
    • American Nurses Credentialing Center (ANCC)
    • Oncology Nursing Certification Corporation (ONCC)
  • Bilingual (Spanish-speaking) is a plus.
  • Experience with EMR/EHR systems preferred.
  • New graduates encouraged to apply.

Work Environment:

  • Primarily provides care in patients’ homes; living conditions may vary.
  • Requires travel within Massachusetts; flexible scheduling, including possible evenings and weekends, to meet patient needs.
  • Collaborates with a multidisciplinary healthcare team.

Salary: $135,000-$200,000
