Job Summary

We are seeking an experienced Director of Care Management & Remote Patient Monitoring (RPM) to lead the continued growth of our House Calls clinical support programs. This leader will oversee a multidisciplinary team responsible for delivering proactive, patient-centered care while ensuring the successful execution of CMS care management initiatives, including Advanced Primary Care Management (APCM), Chronic Care Management (CCM), Principal Illness Navigation (PIN), Behavioral Health Integration (BHI), and Community Health Integration (CHI).

The Director will be accountable for aligning these programs with organizational goals in patient outcomes, quality measures, and financial performance. In addition, this role will drive the expansion of our Remote Patient Monitoring program, ensuring patients benefit from technology-enabled care coordination and providers have the tools and support needed to achieve high-quality results.

Responsibilities

 

Key Responsibilities:

Leadership & Strategy

  • Provide vision and leadership for Care Management and RPM, ensuring integration into Essen’s broader population health strategy
  • Mentor, coach, and build a high performing team culture that emphasizes accountability, patient-centered care, and professional development.
  • Translate organizational strategy into operational roadmaps and measurable results.
  • Oversee annual budget, staffing models, and resource allocation.

Program Management

  • Oversee the design, implementation, and ongoing improvement of care management workflows, ensuring compliance with CMS guidelines.
  • Manage the daily operations of the RPM program, including patient enrollment, monitoring protocols, escalation pathways, and reporting.
  • Collaborate with IT, operations teams, And External Vendors to ensure systems, tools, and data support efficient and effective care delivery.

Quality & Performance

  • Monitor key performance indicators, including patient outcomes, quality measures, and program financial performance.
  • Drive continuous improvement initiatives to enhance care coordination, patient engagement, and provider satisfaction.
  • Ensure compliance with all regulatory and payer program requirements.

Collaboration & Communication

  • Serve as the primary liaison between care management teams and providers, ensuring timely communication and issue resolution.
  • Foster cross-functional collaboration across clinical operations, quality, IT, and finance teams.
  • Provide regular updates and reporting to senior leadership on program performance and opportunities for growth.

Qualifications

 

Qualifications

  • Bachelor's degree in nursing, Health Administration, Public Health, or related field required; Master's degree preferred.
  • 7+ years of progressive experience in care management, population health, or clinical operations, with at least 3 years in a leadership role.
  • Demonstrated experience managing CMS care management programs (CCM, PCM, BHI, CHI, APCM).
  • Experience implementing or overseeing Remote Patient Monitoring programs strongly preferred.
  • Strong leadership, organizational, and change management skills.
  • Excellent communication and interpersonal skills with the ability to influence across clinical and operational teams.
  • Proficiency in data-driven decision-making and program performance analysis.

 

Compensation & Benefits

  • Base compensation of $70,000-90,000 with annual bonus based on performance
