Job Summary

Act as the primary point of contact for assigned patients across all enrolled Care Management Programs.
o Coordinate with support staff for tasks such as referrals, DME, labs, and diagnostic orders.
o Creation of care plans
o Identification of Gaps in cares.
o Track patient progress for all programs (APCM, CHI, PIN, BHI) and update records in eClinicalWorks (eCW)/Ecares.
o Monitor adherence to care plans and ensure timely follow-ups.
o Proactively identify issues across programs and escalate unresolved or complex cases to the care manager.
o Communicate with patients and caregivers to provide updates on all programs.
 

Responsibilities

o Maintain accurate documentation of services provided, including time tracking for program billing.
o Collaborate with support staff to ensure smooth task execution.
o Participate in quality reviews and team meetings to share insights

Qualifications

Experience:
1. 1-2 years of direct patient care experience
2. Experience in Care Coordination or Case Management
3. Experience with Remote Patient Monitoring
4. Familiarity with EHR
Rate: $20.00
