Job Summary

The Vice President of Population Health is responsible for designing, implementing, and managing population health strategies to improve patient care and reduce health disparities. The Vice President works collaboratively with healthcare providers, community organizations, and payers to develop care models that enhance patient outcomes and optimize resource use. This role involves leading data-driven approaches to monitor, assess, and improve population health initiatives.

Responsibilities

Program Development & Implementation:

  • Lead the design, implementation, and management of population health initiatives aimed at improving health outcomes for a defined population.
  • Develop and implement care coordination and case management programs targeting chronic disease management, preventive care, and health promotion.

Data Analysis & Reporting:

  • Utilize population health analytics and tools to track health outcomes, identify gaps in care, and assess program effectiveness.
  • Analyze health trends, risk factors, and outcomes to provide actionable insights and inform decision-making.
  • Prepare reports on population health metrics and outcomes for leadership and stakeholders.

Collaboration & Partnership:

  • Collaborate with healthcare providers, community organizations, public health entities, and payers to implement evidence-based interventions.
  • Serve as the liaison between clinical teams, patients, and external partners to improve care coordination.
  • Work with multidisciplinary teams, including physicians, nurses, social workers, and data analysts, to address the healthcare needs of the population.

 

Risk Management & Cost Control:

  • Develop strategies to mitigate health risks and manage high-risk patients within the population.
  • Implement cost-effective interventions to reduce hospital readmissions, emergency room visits, and other high-cost healthcare services.

 

Quality Improvement:

  • Lead continuous improvement initiatives focused on enhancing quality of care, patient safety, and health outcomes.
  • Ensure compliance with federal, state, and organizational regulations related to population health.

 

Education & Engagement:

  • Educate and empower patients to engage in their care and manage chronic conditions effectively.
  • Develop educational materials and programs for patients and community members to promote healthier lifestyles and preventive care.

 

Policy & Procedure Development:

  • Establish and enforce policies and procedures that support population health management and care coordination.
  • Stay informed of changes in healthcare policies, regulations, and best practices that impact population health management.

Qualifications

  • Bachelor’s degree in public health, healthcare administration, nursing, or related field (Master’s preferred).
  • 5+ years of experience in population health management, care coordination, or related healthcare management roles.
  • Strong understanding of population health concepts, healthcare delivery systems, and value-based care models.
  • Experience with data analytics and reporting tools used for population health management (e.g., EHRs, data dashboards).
  • Excellent leadership, communication, and project management skills.
  • Familiarity with public health policies, risk management, and regulatory requirements.

 

Skills:

  • Proficiency in population health management tools and software.
  • Strong analytical skills with the ability to interpret complex data sets.
  • Leadership and team management experience.
  • Excellent written and verbal communication skills.
  • Ability to collaborate with diverse stakeholders, including healthcare providers, community leaders, and policymakers.

 

Work Environment:

  • This position may require occasional travel to meet with community partners, healthcare providers, or attend conferences related to population health management.
