Job Summary

Job Summary: The Physician Assistant (PA) specializing in house calls provides high-quality medical care to patients in their homes, particularly focusing on individuals with chronic illnesses, limited mobility, or barriers to accessing traditional healthcare. This role emphasizes patient-centered care, effective communication, and collaboration with other healthcare professionals.

Responsibilities

Responsibilities include:

  • Patient Assessments: Perform thorough evaluations of patients’ health status, including physical examinations, health histories, and medication reviews during home visits.
  • Diagnosis and Treatment: Diagnose medical conditions and develop personalized treatment plans, prescribing medications and therapies as appropriate.
  • Care Coordination: Collaborate with physicians, nurses, and other healthcare team members to ensure comprehensive and continuous care for patients.
  • Patient Education: Educate patients and their families on disease management, medication adherence, preventive care, and lifestyle changes to enhance health outcomes.
  • Documentation: Maintain accurate and detailed records of patient encounters, treatment plans, and follow-up care in electronic health records.
  • Follow-Up Care: Schedule and conduct follow-up visits to monitor patients' progress, adjusting treatment plans as necessary.
  • Emergency Management: Assess and address urgent medical needs during home visits, coordinating referrals and additional support when required.
  • Community Outreach: Engage in community health initiatives to promote awareness of home healthcare services and available resources.

Qualifications

Job Requirements:

  • Master’s Degree required
  • Active New York license as a Physician Assistant
  • Currently New York Controlled Substance and Drug Enforcement Registration
  • Master's degree
  • Graduate of an approved PA Program and a post-graduate training program in surgery or at least one-year equivalent work experience
  • Current Certification by the National Commission on Certification of Physician Assistants (NCCPA)
  • Emergency Medicine / Family Practice/Acute Care
  • DOT, BLS, ACLS Preferred
  • Bilingual a plus - Spanish
  • Proven work experience as a physician assistant or nurse
  • Knowledge of EMR Systems preferred

Work Environment:

  • Primarily operates in patients’ homes, which may vary in condition and accessibility.
  • Requires travel within a designated geographic area, with flexibility to accommodate patient needs, including potential evening and weekend visits.
  • Collaborative work with a multidisciplinary team may occur remotely or in various healthcare settings.

Salary: $115,000-$130,000
