Job Summary

Job Summary:

The General Manager is responsible for administering the daily operations for the CMA programs within the health home division. The incumbent supervises the Care Coordination supervisors and monitors the quality-of-service deliveries using the standard of care and workflows. The responsible general manager guides, participates and consults with all care management disciplines and continuously trains the program staff to practice the following integrated care model, including person-centered and recovery-oriented approaches.

Responsibilities

  • Administers the daily specialty program operations and provides supervision to the assigned Care Coordinator Supervisors. 
  • Reviews and evaluates the current program protocols and workflows, (ie. diligent search) to ensure that staff practices the model of care associated to person centered and recovery model.
  • In concert with the QI department and Associate Director, evaluates the effectiveness of the service delivery to all enrolled members and identifies targeted areas needing improvements. 
  • Conducts chart review activities and provides, with the CC supervisors, instructional feedback to improve care managers' clinical intervention and work performance.
  • Provides weekly supervision and coaching to the care management supervisors; ensures that all staff receives quality supervision. 
  • Monitors all quality metrics and ensures staff are engaged in the appropriate number of encounters daily and weekly to meet departmental benchmarks.
  • Reviews members’ charts to monitor staff compliance with assessments, plan of care, HCBS assessments, care conference, encounter notes (DA®P), annual consents, CES tool.   
  • Consults, and if necessary, participates in aiding the team on specific crisis interventions.  
  • Periodically, leads in-service training to improve care management staff’s knowledge and efficacy on specific program standards, such as diligent search, safety in the field- home visits.  
  • In partnership with the leadership team, meet the goal and objective for the specialty program. 
  • Participates in planning meetings to report and track program deliverables and outcomes. 
  • Conducts meetings with care management staff to provide updates on program performance in quality service delivery, QI and department goals.
  • Performs all other duties as assigned.

Qualifications

  • Masters’ degree in Mental Health Counseling, Social Work, Creative Art Therapy, Vocational Rehab Counseling or Nursing. Current license to practice the appropriate disciplines in NYS.
  • At least two years’ experience working in behavioral health or clinic environment, with at least three years of care management experience working with a special population, and two years supervisory experience is required.   
  • Strong knowledge and specialty in working with homeless or formerly homeless populations with co-occurring disorders, including HIV, chemical dependency, severe mental health disorders, and other chronic conditions.  Knowledge of the Collaborative Care Model.
  • Strong knowledge of social, behavioral health, substance use, harm reduction and person in recovery services in New York City. 
  • Proven track record in managing and overseeing programs with more than five hundred members. 
  • Collaborate and work well with others to promote the interdisciplinary approach. 
  • Excellent written and oral communication skills; proficient in Spanish or other languages.  
  • Familiarity in electronic health records (FCM, eCW), Medicaid billings and other regulatory agency web portal systems – Maven, CARES, CAIRS, MAPP, MAS, and PSYCKES.
  • Knowledge of regulatory agencies, including NYCDOHMH, NYSDOH, HRA DHS, and OCF, and SAMHSA.
  • Demonstrates the level of computer literacy to operate common software, include office suite, excel, PowerPoint and Microsoft words

Salary: $70,000-$75,000
