Overview

Our growing Health Home Division is a "Virtual Home" for Medicaid recipients with chronic health conditions. It is a care management model where all individual caregivers communicate to address patients' needs comprehensively. Essen's health home division has a team of care coordinators who connect to a patient's care team and connect members to the social services they need, including housing, transportation to appointments, medication reminders, educational courses, financial support, and more.

Job Summary

Job Summary: The Intake Supervisor manages and supervises the Outreach Specialists team. The incumbent will individually monitor the team's performance and productivity so the team meets the daily and monthly targets. She will review and audit all consents to ensure the team maintains the health home standards. The Intake Supervisor will manage the workflow, create schedules and delegate tasks as required. Also, the Outreach Supervisor will assess the work performance of the outreach specialists and document work areas that need improvement.  

 

Responsibilities

Responsibilities include:

  • Provide ongoing individual supervision to the assigned Outreach Specialists.  
  • Perform periodic performance reviews of the assigned team members to provide guidance and support. 
  • Monitor and observe staff performance during house calls, office, and specialty clinic hours and provide ongoing feedback to improve the outreach specialist's efficacy.  
  • Administer the KPIs to monitor the individual and team's performance in meeting the daily and monthly targets. 
  • Perform quality reviews of all enrollment encounter notes and required documents to ensure compliance with the lead health homes. Approve all new enrollments after determining the appropriateness of the members.
  • Collaborate with the enrollment supervisor to monitor each member's outreach and enrollment process. 
  • Ensure the assigned outreach specialists comply with lead health home outreach and engagement standards.  
  • Train the outreach specialists to increase their efficacy in performing their duties and responsibilities.  
  • Monitor outreach and enrollment process for each patient from outreach to enrollment.
  • Collaborate with internal and external stakeholders, including house calls, office and specialty offices, and CBO providers to increase Essen's visibility and health home enrollment.  
  • Work with other outreach teams to meet Health Home outreach and enrollment goals and objectives. 
  • Conduct quality reviews and patient satisfaction to ensure outreach and enrollment standards adherence.  
  • Follow best practice and clinical standards and adheres to departmental and State guidelines about Health Home.
  • Perform all other duties or actions as required. 

Qualifications

Job Requirements:

  • Master's degree in Social Work or Mental Health Counseling.  
  • NYS license in Social work and mental health counseling are preferred.   
  • At least two years of experience performing intake in a clinical environment. 
  • At least one year of supervisory and training experience is preferred.  
  • Excellent written and oral communication skills required.
  • Proficiency in Spanish and other languages is preferred. 
  • Ability to multi-task well while maintaining a positive "can do" attitude.
  • Demonstrated ability to manage a team in a fast-paced environment while building and enhancing team productivity.
  • Demonstrated professionalism, leadership skills, and the ability to develop, direct, and support staff.

Salary: $70,000-$75,000
