Job Summary

Position Title: Assistant Vice President in Brooklyn Health Home Operations


Job Summary: The Assistant Vice President Brooklyn Health Home Operations overseas and manages our Brooklyn Health Homes Department. The AVP is responsible for supervising the Director of Operations and Associate Director to implement strategic and the overall initiatives to achieve outstanding monthly performances. The AVP will work with the Health Home leadership to ensure the mission and objectives by leading a team of supervisory staff and collaborating with the other Essen department to achieve operational excellence.

Responsibilities

  • Collaborates with senior leadership to develop and implement strategic enrollment plans
  • aligned with Essen's and the lead health homes' mission and goals.
  • Works with the management team to research and analyze data and trends to inform
  • decision-making and develop outreach and engagement strategies.
  • Prepares and reviews reports and presentations on enrollment trends and outcomes and
  • provides recommendations to senior leadership.
  • Works with the team to develop and implement comprehensive outreach plans, strategies,
  • workflows, policies, and procedures that align with Lead health homes and DOH Health
  • Home's regulatory requirements.
  • Oversees the operations of the outreach and enrollment department to monitor the quality
  • of outreach and engagement and compliance with billing practice.
  • Ensures the timely and accurate processing of membership enrollment.
  • Collaborates with the leadership team to develop outreach and engagement strategies to attract and increase membership enrollment to diverse and qualified health home
  • Enhances the enrollment experience by implementing efficient and person-centered
  • outreach and enrollment processes.
  • Hires, trains, supervises, and evaluates Key staff members while promoting professional
  • growth and development.
  • Fosters a culture of excellent customer service, ensuring effective communication and
  • support to potential members, legal representatives and other internal stakeholders.
  • Fosters strong relationships with the offices, house calls, and external partners to enhance
  • membership enrollment initiatives.
  • Remains up-to-date with federal, state, and institutional policies and regulations related to
  • Health Home, ensuring compliance and implementing necessary changes.
  • Oversees departmental QI audits and compliance reviews related to outreach and
  • enrollment functions.
  • Collaborates with the legal and compliance teams to ensure adherence to relevant laws,
  • regulations, and best practices.

Qualifications

  • Master's degree in Social Work, Health Administration, Business, and related fields.
  • Minimum of 10 years of progressive experience in health care, non-profit communitybased, or for-profit institution.
  • Strong knowledge of health home outreach modalities to maximize membership
  • She has demonstrated leadership and supervisory experience, with the ability to
  • effectively manage a diverse team.
  • Excellent analytical and data interpretation, and management skills.
  • Strong strategic planning and project management abilities, with the capacity to prioritize
  • and manage multiple initiatives.
  • Strong communication and interpersonal skills, with the ability to collaborate effectively
  • with various stakeholders.
  • Knowledge of emerging trends and best practices in outreach and engagement strategies.
  • Ability to adapt to changing environments and make informed decisions in fast-paced
  • health care and HH landscape.

Salary: $100,000-$120,000
