Overview

Our growing Health Home Division is a "Virtual Home" for Medicaid recipients with chronic health conditions. It is a care management model where all individual caregivers communicate to address patient's needs in a comprehensive manner. Essen's health home division has a team of care coordinators who function as connectors and coordinators to a patient's care team and connect members to the social services they need, including housing, transportation to appointments, medication reminders, educational courses, financial support and more.

Job Summary

The Assessor is responsible to perform intake screening on all incoming members. The incumbent will utilize internal and state operating standards to screen, evaluate members’ needs based on eligibility, qualifying condition and appropriateness.  The assessor will also complete the DOH’s initial appropriateness within the allotted timeframe (30 days).

Responsibilities

As part of the outreach and intake team, the assessor will:

  • Build relationship and collaborate with Outreach Specialists and other Essen’s divisions to accurately perform intake screenings on all new enrolled members.
  • Complete all intake screenings timely. Evaluate and identify members’ appropriateness based on chronic conditions and needs.  Assess situations that may be potentially harmful to the client. Collect member’s information for enrollment encounter notes and initial appropriateness.
  • Commute and see potential members in their home and community, as well as in-office
  • Document all outreach and enrollment encounter notes in the lead health home system (FCM or BAHN) and electronic health record system (eCares).
  • Maintain excellent communication and effective working relationships with house call and office staff – including medical assistant, office managers, physicians and other health care providers.
  • Complies with all Essen’s CMA, DOH and LHH standard of outreach and enrollment practices. 
  • Work as a team with the department to ensure the delivery quality of care services is provided to the member in the outreach and enrollment stage.
  • Participate in all supervision, training, and staff meetings.
  • Perform any other duties as assigned.

Qualifications

 

  • Minimum of a Bachelor’s degree in social science human services or master’s degree in a field of human services, social work and social sciences.
  • Minimum of one-year professional experience working in outreach or care coordination.

Hourly range: $18.00- $25.00
